High School Admission Procedure

(SPUS GS Graduates)

Step 1: High School Booth

Requirements:

• Original Report Card/Form 138-A
• Photocopy of the following:
• Birth Certificate
• Baptismal Certificate
• Certificate of eligibility from Dep Ed
• If the enrollees is non-Filipino Citizen
• Fill up enrollment form

Step 2.Registrars Office

• Get Student Number

Step 3.Finance Office

• Pay Good Moral Certificate fee (php 50.00)
• Pay enrollment fee (not less than php 1,000.00)

Step 4.Guidance Booth

• Long brown envelop
• Two pieces 1x1 pictures
• Clearance for Good Moral signed by Finance Office
• Fill up Personal Data Sheet (GCO-F-01 (001))

Step 5.High School Booth

Requirement:

• Get Official enrollment slip

NEW ENROLLEES

Requirements:

• Original Report Card/Form 138-A
• Photocopy of the following:
• Birth Certificate
• Baptismal Certificate
• Parents Marriage Certificate
• Fill up enrollment form

Step 2.Registrars Office

• Get Student Number

Step 3.Finance Office

• Pay testing fee (php 250.00)
• Pay enrollment fee (not less than php 1,000.00)

Step 4.Guidance Booth

• Long brown envelop
• Test Receipt ( Php 250.00)
• Two pieces 1x1 pictures
• Original Certificate of Good Moral
• Fill up Personal Data Sheet (GCO-F-01 (001))

Step 5.Pencil &Eraser/ Ball pen

• Take entrance test and get test slips (GCO-F-007-(001))

Step 6.High School Booth

• Test slips (GCO-F-007-(001))
• Get Official enrollment slip
